POSITION TITLE: Nurse Navigator-Oncology/Breast Care
POSITION LOCATION: California
The Nurse Navigator works collaboratively with an interdisciplinary team and is responsible for providing comprehensive and age appropriate care/consultation to patients during periods of illness and wellness. This position provides support and guidance, linking patients and their families to healthcare services that meet their needs and serving as a communication bridge among and between patients, families and members of the patient's care team. The Nurse Navigator coordinates resources into a seamless model of access, care and support that benefits patient, family members and participating physicians. Additionally, this position tracks reports for each patient's clinical portfolio and navigation activity while monitoring clinical quality metrics. The candidate will have demonstrated understanding of patient-centered care practices that support the whole person, and ability to personalize communication so that each patient has access to timely, essential information to make informed choices.
Current CA RN license Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on the unit. Knowledge of growth and development over the life span. Patient navigation experience preferred; breast patient navigator certification desirable. Demonstrated use of clinical skills and expertise in site specific cancer diseases, treatment options, research protocols and psychosocial impact of disease on recovery.
Prefer 3 years’ experience in an Outpatient Center / Acute Care Hospital. Graduate from an accredited nursing program required. BSN Required, MSN Preferred. Focused on Patient Centered Cancer Care. Prior oncology experience preferred; oncology certification desirable.
